Community Information Exchange

A Community Information Exchange (CIE) connects health care, human, and social services partners to improve the health and well-being of communities. OHLC and technology partner Unite Us (UU) are launching Connect Oregon, an emerging CIE effort in Oregon and SW Washington.

CONNECT OREGON

Health care and social service providers will come together through Connect Oregon to take a more coordinated approach to address the social health of individuals they serve. Partners will work together, on a common technology platform, to send and receive electronic closed-loop referrals, address people’s social needs, and improve health across communities.

Connect Oregon is sponsored by numerous partners and led by Samaritan Health Services/IHN CCO, Kaiser Permanente, Central Oregon Health Council, PacificSource, CareOregon, and Health Share of Oregon. Oregon Health Leadership Council is a convening partner for the network.

Conversations continue with other health care partners to help fund this statewide effort. 

Connect Oregon operational partners include 211info as a coordination center, social service organizations as network referral partners, and an Oregon-based Unite Us community engagement and support team.
